All formwork must be designed by a qualified person to support concrete and construction loads. Formwork plans with calculations must be available at the job site. All formwork must be maintained in structurally sound condition and must not have missing or damaged supports. Shoring must be installed properly and in adequate quantity to support the load. Rebar must have adequate concrete coverage to prevent worker contact with sharp reinforcing steel. When rebar is handled during construction, caps or guards must be installed to prevent eye injuries. Concrete placement must be done safely with adequate communication between pump operators, concrete suppliers, and placement personnel. Ready-mix trucks must not be allowed under raised concrete buckets or other suspended loads. Concrete buckets must be controlled by tag lines to prevent swinging. Personnel must not ride in concrete buckets. Concrete must be placed from safe locations, not from areas where a fall could occur. Hot weather concrete placement requires monitoring of concrete temperature and slump. Rapid concrete setting in cold weather requires adequate curing time. Stripping of forms and removal of shoring must only occur when a qualified person determines the concrete has adequate strength. Premature removal of formwork or shoring can cause collapse. Concrete surfaces must be properly finished to prevent slipping hazards. Removal of concrete from mixer trucks must be done carefully to prevent spills. Concrete spillage must be cleaned up promptly.
